Try to only use either LeftMouseUpAction or ButtonCommand, instead of both LeftMouseDownAction and ButtonCommand, by placing both sets of bangs in the chosen option. And use button images 3 times wide than their height.
Other than that, try loading only this skin, without any JaxCore modules or scripts and preferably without the plugin .dll files that throw errors about not being able to be loaded, just to eliminate any potential interference and attempt to isolate and find out where the issue comes from.
